In consideration of being permitted to purchase goods on credit from Picadilly, I hereby agree to the following terms and conditions:

• That all purchases and other charges will be paid in full within Picadilly’s terms.

• To pay for any purchase made by the applicant or by any persons action on the applicant’s behalf.

• Product may only be returned with prior authorization from Picadilly and may be subject to a restocking fee.

• All Cancellations must be submitted in writing 60 days prior to start ship dates or are subjected to a 20% restocking fee.

• To promptly notify Picadilly of any changes of address or name of this business or any of its principals or directors.

• That Picadilly or an agent thereof may obtain credit, bank and/or personal information as may be required at any time in connection with this credit hereby applied for or any renewals or extensions thereof.

• I give authorization to my bank to release any pertinent banking information to Picadilly or an agent thereof in relation to this credit application in consideration of granting credit and the maintaining of the said credit.

• I consent and authorize Picadilly to obtain and or share information with any credit reporting and or collection agency, or any other source for the purpose of maintaining the said credit.

• Interest of 2% per month (24% per annum) is calculated on past due balances and added to the account on the day following your due date.

The customer hereby declares that all the information provided on this Credit Application & Agreement is true and accurate. I consent and authorize Picadilly to obtain from any credit reporting agency or any other source at any time in connection with the credit hereby requested. I have read this agreement and accept all its terms and conditions.
